About Paragon Community Services - Oconomowoc

Paragon Community Services offer Day Services and Transition Services at locations across the state, include two locations in Green Bay.

Day Services include Adult Day Programs and PAC (Paragon Activity Club - Respite and Camp).

Adult Day Programs are offered to individuals with varying disabilities, with services and activities based on the unique needs of the individual, and focus on community involvement and positive peer relationships. many of the structured activities are centered on developing socialization skills and increasing independence.

PAC is a one-of-a-kind recreational program for children and young adults with special needs. It provides participants the opportunity to socialize in positive and engaging environments while taking part in fun activities, events, and community outings. Skill development, such as social skills or activities of daily living are worked on throughout each day.

PAC Respite is available Monday-Friday, from 3pm-6pm, during the school year.

PAC Camp is available Monday through Friday during all school breaks.


Transition Services offered by Paragon Community Services include Employment Services and a Prevocational Skills Program; both are available in Green Bay.

Paragon Employment Services is a service provider for the WI Division of Vocational Rehabilitation. The goal for each of their clients in competitive integrated employment; they offer a variety of services for consumers.

Empower Prevocational Skills Program Monday is a hybrid of facility and community-based programming that works on skill development to lead to competitive integrated employment. We provide options such as career exploration, community integrated experiences, skill development and training, development of daily living skills, socialization experiences, and pursuit of personal interests. Long term success in employment comes as a direct result of building these skills along with an understanding of personal choice and rights. Program times are 9:00am - 3:00pm, Monday - Friday.
