Northwest Colorado Health supports community wellness through a range of health programs and services helping residents of all ages, income levels and cultures live as healthy as possible. Northwest Colorado Health (formerly Northwest Colorado Visiting Nurse Association) was established in 1964 as a Home Health and Public Health agency in Routt and Moffat counties. We have grown significantly over the decades, expanding our services to include primary care at Community Health Centers in Steamboat Springs and Craig, a wide range of Public Health programs in Routt, Moffat and Jackson counties and Home Health and Hospice services in Routt, Moffat and Grand counties. In addition to our Community Health Centers, we operate The Haven Assisted Living Facility and Community Center in Hayden and the Rollingstone Respite House in Steamboat Springs.
