Here at Diamond Willow we take pride in our person centered care model which is uniquely different from most diagnosis based care systems. With that comes the belief that bigger is not always better. Each setting is designed to be intimate with fewer residents per suite, allowing us to have smaller communities to promote our person centered care model in a truly calm and comforting home-like environment. Our small community settings comprise only 8-16 residents per home who are provided the ultimate care by an integrated team of direct care staff, onsite leadership team, and site-based registered nurses - all of whom work collaboratively to deliver the best assisted living services. This affords us the ability to provide one on one attention in a more positive, healthy atmosphere to each and every individual. It is because of this model that we offer the industry leading staffing ratios of 1:5 during awake hours and 1:13 overnight.
