Our staff at Child & Family Connections focuses its efforts on creating and delivering a quality program that supports the needs of families enrolled in the early intervention system. We practice certain philosophies to facilitate our continued success that include a sense of individualized care, a proactive nature and collaborative partnerships within the community. Our team focuses on delivering care based on the individual needs of the infants, toddlers and families we serve.;Our CFC assists families through the early intervention process with a collection of services for infants and toddlers who have delays in development or are at risk for significant developmental delays. Child and Family Connections is the local system point of entry for all children birth to three.;We are responsible for;processing referrals to the early intervention system;determining eligibility for EI services;developing the Individualized Family Service Plan (IFSP);monitoring ongoing IFSP services;providing transition services;conducting Child Find activities;promoting public awareness.
