We are a nonprofit organization with over 40 years of experience helping adults with long-term care needs live as independently as possible. Our Medicare/Medicaid long-term care programs serve the needs of older adults and adults with disabilities. We coordinate and deliver a full range of supportive services that help them live safely, confidently and with dignity. Community Care has neglected to take care of many of our complaints of the facility that they suggested for my mother with Alzheimers. They are always short staffed and now the person who works with my sister has stated they are unable to help and neither is the facility in getting my mother anti-virals since she tested positive for Covid yesterday. You need the anti virals within 3 days of being diagnosed. They have mishandled my mothers situation on multiple levels. I would not recommend Community Care in Wisconsin to anyone who has a loved one needing help.

Terrible insurance to deal with. I never heard back when I submitted my clinic to be in network to serve their members. We had some back and forth when I sent an application then no one followed up. Whenever I contact to speak with benefits and eligibility the live person just transfers you to a voicemail. Same thing with authorization requests. Once I finally got a hold of one of their case managers for mental health services for a patient--he just said no we dont do that. Bearing in mind I had to make numerous attempts to reach out to him trying every different number I can find for this person and speaking and escalating to the supervisor. Do not sign up for this insurance as a member. Theres MANY medicaid plans to choose from.

It is Shocking the lack of due diligence the community care team puts in to clients. I have repeated requested hand railings for my brother . They have put in a temporary 6 foot ramp for my brother in the garage which most certainly is not code. They could not put in 8 foot because he ran in to the toolbox in the garage. No one has ever come to the house to see how my brother and his in home workers navigate this. It is dangerous and too much physical exertion for some workers to push him up the ramp. AT LEAST COME TO THE HOUSE THey said a ramp In The front would be too costly but dont even show up to see if he is safe
